Terms & Conditions
Effective Date: December 5, 2025


By booking an appointment, purchasing services or products, arriving for an appointment, or receiving any treatment through sarainjects.com or in person at Pure Medical Spa (4801 Golf Rd, Skokie, IL 60077), you (“Client”) fully and unconditionally agree to these Terms & Conditions.

1. Services Performed Under Medical Direction

All cosmetic injectable services (including but not limited to Botox®, Dysport®, Xeomin®, dermal fillers, Kybella®, PRP, sclerotherapy, and similar treatments) are performed by Sara Kadhim, RN, an independent nurse injector (1099 contractor), under the medical license and supervision of the Medical Director of Pure Medical Spa and in full compliance with Illinois law.

2. Independent Contractor Status

Sara Kadhim, RN is an independent contractor and is not an employee of Pure Medical Spa or any supervising physician. Client acknowledges that Sara Kadhim, RN is solely responsible for scheduling, pricing, marketing, and performance of services booked through sarainjects.com.

3. No Refunds – All Sales Final

All payments for services, packages, gift certificates, products, and deposits are strictly non-refundable and non-transferable under any circumstances—including, but not limited to, dissatisfaction with results, complications, allergic reactions, scheduling conflicts, or inability to use purchased services. There are no exceptions.

4. Informed Consent & Complete Release of Liability

Before any treatment, Client will review and sign a detailed informed consent form. By signing the consent form and proceeding with treatment, Client:

  • Voluntarily assumes all risks associated with the procedure

  • Forever releases Sara Kadhim, RN from any and all liability

  • Waives any right to bring claims or legal action against Sara Kadhim, RN personally


5. Full Waiver and Covenant Not to Sue

Client hereby irrevocably releases, waives, discharges, and promises never to sue Sara Kadhim, RN (individually or doing business as sarainjects.com) for any claim, demand, damage, expense, or injury of any kind—whether known or unknown, foreseen or unforeseen—arising from the services provided. This includes, but is not limited to, allergic reactions, infection, scarring, asymmetry, vascular occlusion, blindness, stroke, or death.

6. No Guarantee of Results

Results vary by individual. No specific outcome is promised or guaranteed. Before-and-after photos on sarainjects.com or social media are for illustration only and do not guarantee Client’s personal results.

7. Photography & Marketing Consent

Client grants Sara Kadhim, RN irrevocable, worldwide permission to take and use before-and-after photographs and videos for marketing, educational, and promotional purposes across all platforms (website, Instagram, TikTok, etc.) unless Client opts out in writing at the time of service.

8. Cancellation, Late Arrival & No-Show Policy

  • Cancellations or rescheduling with less than 48 hours notice, late arrivals exceeding 15 minutes, or no-shows will be charged the full treatment fee.

  • A valid credit card is required to reserve all appointments. The card on file will be automatically charged for late cancellations, late arrivals, or no-shows.


9. Age Requirement

 Clients must be at least 18 years old. Government-issued photo ID may be required.

10. Governing Law & Exclusive Venue

These Terms are governed by the laws of the State of Illinois. Any dispute must be brought exclusively in the state or federal courts located in Cook County, Illinois.

11. Severability

 If any part of these Terms is deemed unen Severability If any provision of these Terms is found to be unenforceable, the remaining provisions remain in full force and effect.

12. Entire Agreement

These Terms & Conditions, together with the signed informed consent form(s) and any treatment-specific disclosures, constitute the entire agreement between Client and Sara Kadhim, RN.

By proceeding with any service or payment, you confirm that you have read, understood, and fully agree to these Terms & Conditions.
